The major 3 nursing diagnosis are given below:

Assessment nursing diagnosis intervention rationale evalauation

Subjective data:

Child may verbalizet that he/she has chest discomfort

Objective data:

Child may struggle to take breath normally

BR: 12/min

Goal:

Relief from chest discomfort.

Ineffective cardiac tissue perfusion related to reduced coronary blood flow.
  • Assess the child's condition.(discomfort location, intensity, radiation, duration.
  • Obtain a 12 lead ECG recording during chest discomfort.
  • Administer oxygen as prescribed.
  • Administer medication therapy as prescribed and evaluate the child's response continuously.
  • Ensure physical rest: backrest can be provide, use of bedside commode.
  • Assessment will provide baseline data about child's condition.
  • An ECG during symptoms may be useful in the diagnosis of on going treatment.
  • Oxygen therapy increases the oxygen supply to the heart.
  • Medication therapy will prevent the child from further worse.
  • Physical rest provide comfort and relieves from discomfort.
Child may verbalize that she/ he relieved from chest discomfort. BR: 18/min

Nursing daignosis intervention rationale evaluation

Subjective data:

Child may verbalize that he/she is less frequently voiding urine.

Objective data:

Urine output: 15ml/hr

Decreased cardiac output related to dysfunction of heart muscles.
  • Assess the child's condition by ascultating the heart sound, blood pressure.
  • Monitor hemodynamic parameters to assess cardiac output, volume status.
  • Monitor ECG pattern for cardiac dysrhythmias and ischemic changed.
  • Measure urine output every half hour to 1 hour at first, vital signs.
  • Provide prescribed medication.
  • Assessment will provide baseline data about child's condition.
  • Rising CVP (central venous pressure) and PAWP ( pulmonary artery wedge pressure) may indicate pulmonary edema. Low pressures may indicate need of volume replacement.
  • Taking ECG will help to rule out what the actual problem.
  • Urine output less than 30ml/hr indicates decreased renal perfusion and may reflect decreased cardiac output.
  • Medication like laxative will improve the quantity of urine output.

Child urine output may increase as 30/hr

Child may verbalize that he/she has fear to play with friends due to fatigue.

Objective data:

Child looks dull

Activity intolerance related to decreased cardiac output and fatigue.
  • Assess the child's condition and level of activity.
  • Begin with few minutes of warm-up activities.
  • Avoid performing physical activities outside in extreme weather.
  • Wait 2 hours after eating a meal before performing the physical activity.
  • Stop the activity if severe shortness of breath, pain or dizziness develops.
  • End with cool down activities and cool down period.
  • Assessment of child's condition will provide baseline data to render proper care.
  • These Physical activities will improve child's activity
Child may verbalise that now he/she can play with their friends without any discomfort.
