Subjective data:- my patient says that i am having increased body temperature.

Objective data:- on checking the body temperature, it is 101degree.

Nursing diagnosis:- imbalanced body temperature hyperthermia related to dehydration as evidenced by checking temperature.

Goal:- patient maintains normal body temperature.

Plan of action

Implimentation

Assessed the general condition of the patient that my patient is having increased body temperature of 101 degree farenheit
Checked the vital signs periodically
Provided tepid sponge to the patient
Advised the patient to drink more fluids
Administered antipyretics as per physician order.Eg- paracetamol

Evaluation:- patient maintained normal temperature as evidenced by checking temperature
