Mrs. A. is an 84-year-old woman who was recently discharged from the hospital with a diagnosis that includes an exacerbation of congestive heart failure (CHF). She also has diabetes and hypertension. Mrs. A. was discharged from the hospital on multiple medications and lives alone. Home care services were initiated with skilled nursing care visits, some home health aide support, and orders to include daily tele-monitoring of her vital signs. The telehealth device will remotely monitor Mrs. A.’s blood pressure, heart rate, oxygen saturation, and weight. In addition, the patient will answer customized questions about her disease on a daily basis. This information will then be transmitted daily to the home care agency, where the Telemetry nurse can determine appropriate clinical actions based on the data trends and preset baseline alerts that indicate when set parameters have been exceeded.

1) Develop a work flow for a telehealth nurse to ensure all needs would be covered for a hypertensive patient. From the initial greeting to the end of conversation, must be at least 10 steps.

2) As a telehealth nurse would you have any recommendations to Mrs. A’s physician or any other members of the health care team to provide adequate care? Name at least three professionals with recommendations for each.

As a telehealth nurse I would have some recommendations for this patient to be cared for by different professionals.

a. Cardiologist- I would like to recommend a weekly telehealth assessment of this patient by her acquired cardiologist, as she has a history of cardiac disease this recommendation would help manage her health well.

b. Endocrinologist- I would also recommend Mrs. A to refer to her endocrinologist or general physician for the diabetes monitoring and follow up, this would help her maintain and manage her diabetes and other comorbidities.

c. Dietician- I would recommend Mrs. A to be referred to a dietician for her proper meals and medication routine. This would help her get routine recalls for proper and nutritious meal and medication adminsitration on time.
