In addition, the patient will answer customized questions about her disease on a daily basis. This information will then be transmitted daily to the home care agency, where the Telemetry nurse can determine appropriate clinical actions based on the data trends and preset baseline alerts that indicate when set parameters have been exceeded.

-Live video conferencing, it is a two-way interaction from patient and provider through audiovisual telecommunication.
-Store and forward transmission of health history to health care providers from the patient.
-Remote patient monitoring collects patient health data and transferred electronically to the nurse or health care providers. It helps to track patient vitals and activities.
