1)Mr.jacob is having stoma necrosis..This may occurs due to the decreasing blood flow to the stoma..or it can be related to the surgical procedure which has done.
2)stoma necrosis appears with in five days of surgery.while this time it does not appears like red,moist and shine.it can be dark red,cyanotic,bluish purple,brown or black .stoma will be hard,dry or flaccid.there will be foul smell also present in necrotised stoma.
3)1)post operative assessment by a nurse will aid to improve blood supply to stoma.
2)when swelling in the stoma decreases,nurse should assess the stoma by removing pouch
3)check the stoma for any leakage
4)apply lubricant in each changes
