Purpose

To provide students with the opportunity to assess, plan, prioritize, and implement nursing care for an older adult patient experiencing acute confusion related to a urinary tract infection.

Overview

Bernadette Jackson is an 85-year-old African American female who presented to the emergency department at 1130 Thursday morning with weakness, lethargy, and abdominal discomfort. Medication review and bladder scanning revealed polypharmacy-related urinary retention. An indwelling catheter was placed. Mrs. Jackson was admitted to the inpatient medical unit for rehydration, IV antibiotics, and monitoring.

1.The factors which predispose to the development of urinary tract infection is the indwelling catheter amd the age of the patient

2.The common cause of ITI in acute care settings are

  • Age of the patient
  • Medical conditions
  • Foley's catheterization
  • Women
  • Nosocomial UTI

The role of nurse are

  • Following aseptic procedures
  • Following movements of handwashing
  • Change catheter once in 14 days or as needed before that days
  • Monitor for UTI
  • Encourage to drink plenty of fluids
  • Catheter care twice a day and when there is bowel movements

3.The following patient education is needed

  • To report in case of any lower abdominal pain ,pain at insertion site, fever ,nause and vomiting
  • To complete antibiotics as per order
  • Empty the bag regularly

4.The manifestation are

Confusion is one of the typical indicator and manifestation which can be seen in elderly population ,inability to do daily activities ,extreme fatigue, rectum fullness ,fever very rare.
