Incident report for A patient is 1 year old. She or He received several required immunization including an influenza injection. The medical assistant gave the patient a double dose. This would be a patient error incident report.

In healthcare sector errors and reporting are very common.When any untoward incidents or injury occurs to a patient knowingly or unknowingly it is reported as an incident and documented so called as incident report

Patient error refers to any mistakes happened to the patient on behalf of the medical staff.This is also termed to be medical error ,which includes wrong administration of medication, wrong surgery on wrong patients ,procedures ,investigation ,etc.,Here the patient has experienced a medication error by receiving two doses of vaccine.So it will be mentioned as medication error on incident report.
