1.The discharge process was not done professionally, it is more important to make the patient understand what has to be done after discharge to prevent the relapse of the problems. Here the nurse in order to get the bed free for next patient was in a hurry to explain and was not bothered or concerned regarding the patient's ability to understand. The are of concern are the patients inability to understand the language, discharge without a family member, risk for non compliance with drug, treating a patient with disrespectful behavior.

2.The patient has not understood the discharge instruction.Some of the ways to handle it differently are using a interpreter to explain ,waiting for the son to explain as in ,trying to identify whether the patient understood or not.

3.The population belonging to Americsn Indian, Pavific islanders are the most common and prone people for getting type 2 diabetes. The main reason for this is the immigration, poor diet and exercise

4.The follow up care required for this patient are

• Monitoring blood glucose value in next visit
• Ruling out for any adverse events
• Medication effectiveness
