Claims data is used in healthcare as administrative data for the information regarding patients doctors appointments, diagnosis, treatment, management, health insurance, and billed and paid amount of money in a large scale.

Personal health record is a health record maintained by the patient which provide information including the doctors appointments, lab results, medication information, health information from external sources, patient out come. It is controlled, maintained by the patient themselves.

Electronic health record (EHR) is the health care data which store all health care information and statistics regarding patient care in a digital format which can be shared between health care providers or institutions.

Encyclopedia is not a source of healthcare data. It can provide various medical information.
