1. An organizational chart, in this case, clearly defines the extent of each nurse's authority to diagnose and treat a patient as a way to ensure the most professional care. It also allows other health care workers and patients to know exactly who to deal with in the medical hierarchy.

The purpose of an organization chart is to depict the skeletal structure of the practice, including the functional relationships between, among, and within the specific components. An organization chart provides a point of reference and improves the flow and direction of communications.

2. Physician history - is the part of the sheet in which physician writes the history of the patient .

Progress notes - is the part where the physician writes about the patients disease progress .

Medical order - is the portion of the chart where medication orders prescribed by physician is entered .

Laboratory data - is the part where lab reports are entered

Operating room report - is the part where patients operative notes are written .
