The CMS 1500 is a standard claim form used for the medical claim billing. This form has red ink on white paper. This form is used by the suppliers and physicians. It was initially developed by the CMS (Centres for Medicare and Medicaid). However, it is now used by all the insurance carriers.

Item number 1 to 13 in the form refers to the patient and insured information. This part of the form requires information of patient's name, address, employment status, the condition of the patient and the insurance plan. Further, the insured's details are also in the form following the patient details. The relation of the insured and patient is also mentioned in the form. The bottom columns are to be signed by both the insured and patient.
