HIE helps to ensure to provide the patient’s each and every data including their past medical history whenever they visit the practitioner. For the improvement of the quality of health care electronic exchange of health information has got a vital role. It helps to avoid or to reduce medication or medical errors. For these reasons this practice started to implement in the new prescription. This will make the pharmacist have the information technology tools to be the member of health care team. By the use of evidence based practice principles and the HIT the pharmacist will collect, document and prepare the information for the exchange. The work flow includes collection of medications related patient information, assessment of the medication related issues, devolopment of the patient oriented medication care, and followed by the implementation and monitoring of the patient transition as needed. . The most important benefits of this system is the care provider understand more about the patient , enhance decision making and helps to provide cost effective health care.
