Fecal impaction

1.The intervention which can implemented by the nurse are

• Encourage patient to mobilize out of bed or to be active. This can improve the function of bowel movements
• Provide a fibre rich diet
• Increase the fluid intake in the diet

2.If fecal impaction does not relieve with enema and laxative manual evacuation can provide a successful outcome for the patient. If this also does not works then a laparotomy feval removal is done by a surgeon

3.The education which can be given to the patient are

• Encourage patient to be active
• Include adequate fluid and fiber in diet
• Take laxatives as per order
• Use of vegetable oil in empty stomach is also advisable in these types of patients to relieve constipation

Stool specimen collection

1.Nursing intervention

• Monitor the vital signs of the patient, as bleeding can cause hypotension ,arrhythmia
• Watch for signs of anemia like fatigue, paleness due to bleeding
• Provide trendlenburg position if patient is hypertensive
• Administer oxygen in case of hypoxemia

2.The nursing considerations when obtaining a hematocrit sample are

• Ensure the patient is not anxious
• Collect adequate sample from vein
• Finger prick test should e done slowly and gently
• Sample should not be heamolysed
