Vancomycin through refers to the measurement of tte drug concentration in the blood after its administration. Usually the blood has to be withdrawn after administration of four doses or just before administration of the fifth dose. This trough test is possible only when the patient is given this medication twice or thrice a day.

Here the patient has a frequency of 12 hours means twice a day

The last dose was received at 1500 (3:00 pm ), as it is a BD dose the next dose timing falls by 3:00 am of the next day. So the blood sample has to be drawn before administering the next dose .Ideally 30 minutes before the dose.So the blood has to be drawn at 2:30am

10.Lisinopril is indicated to treat hypertension. It is recommended for long term usage in order to control high blood pressure. So it cannot be administered as and when needed,as a single dose or at once.

A standing order is one which is mentioned by the provider about a particular medication to a patient to treat the condition.Here the nurse is ordered to carry the order for administration of the drug .
