These data is available to all patients whose physicians use EHR .The changes and updation made in the EHR aresimultaneously recorded  in the PHR .The patient entering the signs and symptoms themselves gives a more diversified way for a physician to plan for care and encourages the patient to access health care and improve the quality of care delivered.The paper records are an age old practice which can be destroyed easily or lost.The data in the PHR even if deleted,lost or manipulated can be easily identified and restored.It can improve communication between patient and their primary provider not their specialist.
