The precautions used for checking dosages of anticoagulant drugs are

  • Monitoring the blood values for Prothrombin time / International Normalized Ratio(PT/ INR)The dosage is decided upon the patient condition in case of thromboembolism ,it should range between 2 to 3 .In case of high risk patients it should range from 2.5 to 3.5
  • The INR level has to be monitored atleast six times before starting anticoagulant like warfarin.
  • 4 Anti -Xa LMWH ,the factor should range between 0.5 to 1.2 IU/ml. If this level is in the desired range then as per guidelines the low molecular weight heparin can be started
  • Whole blood clotting time ,activated clotting time is to be assessed
  • Platelets to be aseesd from complete blood count
