ANSWER- A) nurse should determine duration, frequency , pattern and severity of diarrhoea.

  • Nurse should ask about stool characteristics such as stool is watery, contain blood and fatty stool.
  • Should ask about any previous history of gastrointestinal disease.
  • should do physical examination, examine color of eyes, texture of skin , palpate the abdomen

B) Laboratory test-

  • CBC (complete blood count)- to test the haemoglobin level in body, causes of diarrhoea
  • Stool test (stool for occult blood)- to detect blood in stool , RBC
  • Sigmoidoscopy or colonoscopy
  • ultrasonography

C) Nursing diagnosis-

  • fluid volume deficit related to disease condition
  • imbalance nutrition less than body requirement related to disease condition
  • hyperthermia related to disease condition
  • impaired skin integrity related to fluid volume deficit
