“potential preterm labor related to previous preterm labor as evidence by previous preterm at 21 weeks (demise)”

Risk for/Potential preterm labor related to previous history of labor at 21 weeks.

Risk for/Potential preterm labor related to previous delivery of baby at 21 weeks.

When the diagnosis is in the potential or risk form then the evidence is not necessary.For risk or potential diagnosis can write only the related factors and no evidence.

Risk diagnosis are the the problems the patient may or may not be faced.When the problem is not faced  you cannot provide evidence.Other diagnosis are the problems faced by the patient.you are finding that problem by any factor that you are writing as evidence.Example:Pain related to uterine muscle contraction as evidenced by patient verbalization .Here the patient is experiencing pain that is the problem you have find it by patient verbalization that is evidence.
