Today, in your long-term care rotation, you have been assigned BM, an 85 year-old-female. Her scheduled medications include hydrochlorothiazide, losartan, and simvastatin. Her prn medications include docusate sodium, bisacodyl, and polyetheline glycol. You come by to see her right after she finishes lunch in hopes that you can take her for a walk. In the team huddle this morning, it was noted that BM sits in her room watching TV quite a bit. As you remove her lunch tray you notice that she has eaten her grilled cheese sandwich (minus the crusts), drank her milk, and eaten most of a cookie but has left the salad and applesauce. She asks you to bring her coffee and, “One of those pills I take every day so that I can go to the bathroom.” After some discussion, you realize this is the bisacodyl.

1. What questions will you ask her about her diet?

2. What recommendations could you make to LS and the care team to improve her gastrointestinal health?

An eighty-five-year-old widow has been in a long-term care facility for six months. She was admitted with severe osteoarthritic and osteoporotic pain. Her family initiated her placement because she was overmedicating herself at home, confusing her medications, and not eating adequately. She has been a “complainer” for years, the family reports. The family and physician are reluctant to give her strong pain medicines; they don’t want her to be “doped up” like she was at home. She currently takes two...
