1) Terry is depressed due to his illness and so he is comparing himself with other folks and is thinking that he is suffering as he is not as religious as his other folks. It doesn't mean that he is not spiritual but he is only using a defence mechanism.

2) "I might as well die right now" This sentence of Terry suggest that he is experiencing total distress.

3) Illness forces an individual to blame his negligence towards regious practice, and thinks that God is angry as he is not spiritual.

4) spiritual assessment will help to know whether Terry has any superstitious believes and can be assisted to overcome it.
