2. Case Scenario: A five year old presents with high temperature (> 101), oxygen saturation of 78%, and breathing that is labored. His breathing could be heard throughout the ER. The mother states he has had a cold with some mucus in his lungs but has had few problems. The family has been sick with colds. The physical history of this patient shows a well-nourished, functioning child with no history of pulmonary issues. He has had some allergies that have been controlled with over the counter antihistamines. The child appears scared and mom is very worried.

1) Asthma is characterised by inflammation of brochia with increased production of mucous secretion. Early symptoms of asthma happen just before beginning of asthma. They are not severe enough to stop. By treating these symptoms we can prevent asthma from getting severe. Features of early asthma include frequent cough especially at night, shortness of breath, feeling tired, upset, change in lung function, signs of cough cold or allergies.

some people with asthma may have extended periods with no symptoms, interepted by periodic worsening called as asthma attacks. The late asthma symptoms include bronchospasm, severe wheezing, chest pressure. The severity of asthma then escalate rapidly so it is important to treat before it worsens.

2) croup and epiglotitis are both infections of ipper airway. Croup is inflammation affects larynx and trachea and bronchiole. Epiglotitis is the inflammation of epiglottis. Croup is mild infection that can be treated in home and cures within few days. Epiglotitis can occur in any age and can sometimes be severe. Coughing and absence of drooling in case of croup and drooling with absence of cough in case of epiglotitis is the interesting fact.

Croup causes swelling of larynx, trachea and bronchiole due to infiltration of white blood cells results in partial airway obstruction and breathing difficulties. Epiglotitis is due to the bacterial infection caused by haemophilus influenza type B and they can cause obstruction in epiglottis region results in inflammatory symptoms.
