Case Scenario: A five-year-old presents with high temperature (> 101), oxygen saturation of 78%, and breathing that is labored. His breathing could be heard throughout the ER. The mother states he has had a cold with some mucus in his lungs but has had few problems. The family has been sick with colds. The physical history of this patient shows a well-nourished, functioning child with no history of pulmonary issues. He has had some allergies that have been controlled with over the counter antihistamines. The child appears scared and mom is very worried.

Case Scenario: Your patient has asthma, newly diagnosed. His symptoms are shortness of breath, oxygen saturation of 60% on 3 liters of oxygen, anxiety, and loud breathing. He had a respiratory treatment earlier but has awoken with some severe symptoms.

1) Airway resistance is the opposition of the air to flow which is caused by force of friction. It is increased at low lung volume due to reduced airway diameter or during forced expiration. Chronic obstructive pulmonary diseases and asthma increases airway resistance. The child is presented with oxygen saturation of 78% and the breathing difficulties. The airway obstruction is identified with a sign of change in sound of chid cry or the breathing sounds. Medication like atropine can prevent the airway resistance. This child should receive the medication according to the severity of the health conditions.

2) various acid base disorders are associated with asthma patients. Airway hyper responsiveness can lead to hyper ventilation with increased renal bicarbonate loss. This will result in hyperchlorimic acidosis which is a clear effect of blood acidosis in asthma attacks. Hypercapnia characters severe asthma attacks which produce risk of mechanical ventilation.

Careful evaluation of acid base disorders in asthma will giva a diagnostic approach to pathological conditions.
