Case Scenario: A 75-year-old gentleman was referred to an orthopedic physician because of shoulder pain. Upon examination, the physician finds that the man has had shoulder pain for more than five weeks. However, he doesn’t feel the pain when moving his shoulder. Instead, he feels pain after eating. His abdomen is distended, making him uncomfortable, but he states that he has no pain in this area. He has a history of drinking more than six beers per day for many years.

Case Scenario: Your patient is a 10-month-old boy with cleft palate and cleft lip. The chart states the uvula and soft palate structures are affected, and the defect may extend into the hard palate. The child is fussy a lot of the time and appears malnourished.

This gentle man is suffering cholecystitis.

Cholecystitis: This is the inflammation of gall bladder.The signs and symptoms of cholecystitis are

  • Shoulder pain
  • Abdominal distention
  • Nausea, vomiting, fever
  • Severe pain in the upper abdomen
  • Nausea
  • Vomiting
  • Fever

The information provided in the scenario is abdominal bloating , shoulder pain and pain after eating. For the definitive diagnosis, it is necessary to have a CT scan of gall bladder, to identify the blockage of bile ducts, identification of gall stones is very important.A HIDA (Hepatic Imino Diacetic Acid) scan is needed to track the production and flow of bile juice.

The risk factors are pregnancy mother, mother who are on oestrogen pills, Obesity, Heart disorders , elderly people.

Cleft lip and cleft palate are the congenital anomalies that can be treated with operation procedures such as Chieloplasty

Cleft lip affects the upper lip , where as Cleft palate affects the roof of the mouth.Here in the given scenario, the cleft palate involves uvula, soft palate and also hard palate.Chieloplasty can be performed repairing the cleft of all the parts.Children with clefts cannot eat, drink and speak properly.But with the surgery, this can be treated and can lead a normal life.
