Read the improvement opportunity described below: Description of Hospital Nosocomial Infection Problem Mr. Walker, a 65-year old male patient has a history of complicated diverticulitis that resulted in a temporary colostomy. He underwent reversal surgery at City Center Hospital (CCH). His recovery on the surgical unit was uneventful and he was soon discharged home. After resuming a normal diet, Mr. Walker continued to have discomfort in his lower abdomen and he then developed very frequent watery diarrhea. When he found blood in his stool, Mr. Walker decided to go to the emergency department where he reported to the nurses that he felt like he has to sit on the toilet all day. Mr. Walker was readmitted to the surgical unit at CCH for further workup and treatment for probable Clostridium difficile infection. Kate Pederson, an infection control practitioner at CCH, was alerted to Mr. Walker's readmission for treatment of an infection he most likely picked up during his first hospitalization. Kate had recently noticed a spike in hospital acquired C diff infections. Nine cases were identified in the past 8 weeks on the same surgical unit where Mr. Walker underwent reversal surgery and recovery. Cdiff infections is a prevalent hospital acquired infection. Most cases occur in patients who have taken or are currently taking antibiotics, who have long hospital stays, colon surgery, or are immune compromised. C dill forms spores that resist drying, heat, pressure, and many disinfectants. Spores can live up to 5 months on solid surfaces such as bed rails, TV controls, equipment, door knobs, and bathroom fixtures. In addition, spores can dwell on the hands of just about anyone entering the hospital environment including providers, patients, family members house keepers, volunteers, students and other visitors. Preventing the spread of diff. requires careful attention to environmental cleaning, rigorous hand hygiene, and antibiotic prescribing. 2. The hospital infection control committee wants Kate to lead team of people who will conduct a C diff prevention project. Presume you are a member of this committee. Write a project charter for this team by answering the questions below: What is the aim of project? Describe what the committee wants the team to accomplish. • How will success be measured? Identify at least three process or outcome measures. • Who should be on the team? Identify the groups or departments that should be represented on the team.
