Cycle time is the end result of systems and processes (how things are done). Thus, measures of cycle time are outcome measures. In this project you'll create a data collection tool for gathering cycle time data. Instructions 1. Presume you are the quality manager for a health system that includes an outpatient cystic fibrosis clinic. People with cystic fibrosis (CF) require frequent clinic visits to monitor their condition and receive support from a team of CF specialists. 2. The clinic is conducting an improvement project to improve efficiency. One of the first project steps is to gather data about what is happening right now. You are asked to help the project team collect information on patient flow in the clinic. The team wants to know what time a patient arrives at the clinic and what time they leave. In addition, they want to know how long it takes to complete activities that occur while patients are in the clinic. 3. You ask the team for more specifics so you are sure the information gathered will meet their needs. The team responds; stating they want to know the average time between various steps in the clinic process. Based these discussions, you determine the following data elements need to be gathered on each patient during the study time period: • Time of the patient's scheduled appointment Time patient leaves home to travel to the clinic Time patient arrives at the clinic Time patient checks in with receptionist Time patient is shown to the exam room Time nurse finishes clinical measurements (e.g. patient's weight, height, lung function) Time each CF team member comes into exam room to see patient and time they leave (CF team members include: physician, nurse, social worker, nutritionist, physical or respiratory therapy. and others) • Time patient is ready to leave the clinic 4 You decide the best way to gather accurate information for the study is to ask receptionists and patients to record actual times on a data collection form. When a patient registers, the receptionist will ask what time the patient left home and what time they arrived at the clinic. The receptionist will record these times on the form along with the time of registration and the scheduled appointment date and time. The form will then be placed on a clipboard with a pencil attached and given to the patient. The patient will be instructed to record the remaining times. The receptionist will explain why the data are needed and how to complete the form. If the patient does not have a watch one will be provided. Before the patient leaves the clinic, they will be asked to return the clipboard and completed form (and watch, if one was provided) to the receptionist. 5. Design a data collection form that can be used to gather the needed data elements using the method described in 84. The form should be easy to complete and self-explanatory.
