Urinary retention related to bladder obstruction and infection as evidenced by lab investigation results
PLANNING
At the end of 8hours of nursing care, the patient will be able to pass urine.
* Assess the general condition of the patient.
*Check signs of urinary retension.
*Check vital signs and intake output chart.
*Encourage adequate fluid intake and advice to avoid coffeine.
*Observe for cloudy or bloody urine with foul odor
*Provide perineal care and keep dry
*Refer to urinary specialist as indicated
*Monitor urine and blood tests for any infection.
*Provide catheterization.
*Administer medication as prescribed by doctor.
IMPLEMENTATION
*Assessed the general condition of the patient

*Encouraged regarding intake of fluilds.

EVALUATION
After 8 hours of nursing intervention patient was able to void in sufgicient amount of urine with no palpable bladder detention and voiding pattern normalized
