CASE STUDY:  You are the student nurse that has clinical on a cardiac nursing unit.  The clients are assigned a variety of cardiac medications.  As a critical thinking nurse you would assess for any of the following if your client was prescribed a:

  1. Angiotensin Converting Enzyme (ACE) Inhibitor (enalapril, Lisinopril, captopril):
  1. Beta Blocker (atenolol, metoprolol, carvedilol):
  1. Anti-angina Drugs (NTG):  
  1. Positive Inotropic Agents (Digoxin):
  1. Diuretics – If a client is prescribed furosemide or hydrochlorothiazide what type of dietary recommendations will you discuss with the client?

Answer :

Furosemide and hydrochlorothiazide are types of diuretics which use to treat edema caused by disease conditions such as congestive heart failure, renal failure, nephrotic syndrome and renal failure.

Dietary guidelines for a client who is taking Furosemide or Hydrochlorothiazide are;

1)Discuss about the importance of potassium and advice the client to take more potassium rich food

For example :Bananas, prunes, raisins and orange juice

Reason :Hypokalemia is a side effect of these medicines

2)Advice the client to take low - salt and low - sodium diet

For example :Processed foods and ready meals

Reason :Increased salt and sodium inhibit the action of medications.However, if a client is hypotensive, encourage him to discuss with doctor about the amount of salt in diet.

3)Advice to take low fat diet

Reason :Hyperlipidemia is an another side effects of this

medication.

4)Advice the client to take plenty of fluid. Instruct the patient to follow doctor's instruction about the type and amount of liquid.

Reason :These medications make the patient urinate more often and get dehydrated easily.

5)Instruct the client to avoid alcohol

Reason :Alcohol will interact with medication and prevent the medication action

6)While taking this medication advice to avoid eating grape fruit or drinking grape fruit juice. Because this will interact and prevent the action of medications.
