reason ; When a client take this medication, it will increase the concentration level of the plasma and also it will affect the clotting factor, thus lead to have clot formation over the blood vessels. hence it should be clear contra indication if the client had any thrombus problem.

Other condition mentioned in the options are actual indication to use these drugs. Hence the above mentioned condition is contraindicated to use these drugs.

2. Answer ; D) the client was treated for deep vain thrombosis following surgery.

Reason ; If a client is taking OCP, there are so many factor that nurse need to consider before giving these drug. One of the significant history is about the clotting problem.
