Mr. T. was quickly triaged in the ED with a preliminary diagnosis of acute inferior wall MI. The hospital is in a rural area, and the weather is severe. Ground transport to the nearest hospital able to do PCI is 2 hours. Therefore, a decision is made to administer thrombolytics. Within 1 hour, Mr. T. was started on reteplase. Why was Mr. T. treated with thrombolytics rather than transported to the center for a PCI?
If a patient is diagnosed for acute inferior wall myocardial infarction (cessation of blood supply ,oxygen to the myocardium) due to blockage in the coronary artery either right coronary artery or left circumflex artery.
At this time the patient should have undergone cardiac catheterization in 90 minutes( this time period us called as door to vessel open) to save life. If this is not possible within this time limit due to transportation or other reason a immediately thrombolysis to be done. This will break the clot and ensure blood supply for time being to the ventricles and preventing the patient from shock.By this time the patient can be transported to the hospital and can be taken on table for PCI at the earliest.Reteplase helps on preventing heart attack. It repeatedly binds with the thrombus and breaking it making coronary artery potency and preventing major adverse effects .

Because Mr. T. is having an inferior wall MI, what complications should be anticipated while getting him ready for the thrombolytic therapy? What nursing care was done prior to starting reteplase? What is the nursing care associated with a patient receiving reteplase? What complications must be assessed during and after administration of the medication?
