Answer: The process of wound healing includes the primary, secondary and tertiary intentions. The primary intention is to provide better treatment as soon as possible which reduces the risk of infection. Main intention is to keep wound free from microbes.

The secondary intention includes that wounds can not be approximated as sometimes due to microbial infection the healing time of wound gets prolonged.

Tertiary intentions: In this the wounds need to be open for period of time for sutures.

Red wound: The colour of wound red indicates that wound is healing as the red colour occurs due to the flow of the blood to the particular wounded area which starts healing the wound.

Yellow wound: If the yellow colour exists near the wound that means there is some kind of infection and it is not a good sign.

Black wound: Black wound means it has been started to heal, the formation of debris indicate that wound is healing.

The risk factors for pressure ulcer as people who are unable to move due to any kind of injury or surgery for a long time are more prone to pressure ulcer. Poor diet and dehydration may also cause pressure ulcer.
