A patient-centred care system is one where patients can move freely along a care pathway w/o regard to which physician, or other health-care provider, institution or community resource they need at that moment. The system is one that considers the individual needs of patients and treats them with respect and dignity.

The values of patient and family-centred care are incorporated throughout interactions with patients and/or their families

2. Characteristics of patient centered care are :-

- respect for patients preference

- coordination and integration of care

- information and education

- physical comfort

- emotional support

- involvement of family and friends

- continuity and transition
