patient: 7 weeks
diagnosis: Bronchiolitis, respiratory distress, Ventricular septal defect
BMI is 15.78 (Underweight)- discuss the affects this may have on the patient's health
What is the level of Erikson’s theory of psychosocial development? Explain why
Musculoskeletal: (activity, weakness, gait, spinal curve and mobility, mobility of joints) all of these are normal how do to document these finding?
Ventricular Septal Defect is the most common cardiac malformation accounting to congenital heart disease. The size of the left to right shunt is determined by, the physical size of the Ventricular Septal Defect and level of pulmonary vascular resistance. Big Ventricular Septal Defect can lead to heart failure, which will increase the tissue hypoxia and the possibility of chest infections and growth failure.
Erik Erikson proposed a psychoanalytic theory of psychosocial development comprising eight stages from infancy to adulthood. During each stage, the person experiences a psychosocial crisis which could have a positive or negative outcome for personality development. This stage takes place during middle adulthood. Generativity refers making a mark through creating or nurturing things that will outlast an individual.
Patients with isolated Ventricular Septal Defect from Hospital, outpatient and inpatient, studied prospectively for months in regard to the effect of VSD on growth, To reduce the effect of malnutrition and other factors on growth, a random sample attending outpatient clinic for acute simple illnesses were used.
It was found that Ventricular Septal Defect is more common in females, most of the patients were from the city. Failure to thrive were seen in the patients who were under 5 years of age, while severe failure to thrive seen in patients with VSD, and this might be related to the delayed medical and surgical management.
According to the study weight is the most sensitive parameter for studying the effect of VSD on growth and it is greatly affect by the size of VSD with no significant effect of the gender on growth of the VSD patients.
The pulmonary artery pressure assessed the study depending on the pressure gradient through the VSD and compared with the systemic pressure of the patient. In these patients the growth parameters weight, height and head circumference were measured, and various weight scales were used for young patients and older patients.
Both weight and OFC were affected significantly by the presence of VSD, while height had not significantly effected, so weight is the most sensitive parameter for the study of the effect of VSD on growth.
