12. Answer: Health information national trends survey(HINTS). It is the one which conducts cancer related surveys. It contains cancer related information. It is one conducting this survey. While others focus on different issues. BRFSS focus on behaviour issues, YRBSS focus on problems faced by youths and NHIS focus on general health issues currently on house integration issues.

13.precede- proceed model is the only model that works backward from the desired state and includes individual and environmental factors that relates to health. While the other models not working from backward. Hence the answer is precede-proceed model.

14. Health belief model:

This model describes that the individual ' s own thought, ideas, belief describes their own health. One mental health defines their physical health. For example: If an individual has an headache if he is taking it as easy then after sometime the problem resolve. Instead if he related it to serious health problem even the simple headache will not resolve but it makes emergence of various other health problems. That means our own beliefs defines our health. Approaches for grandmother: Consider if she is having arthritis. She thinks that whatever the medicines taken she can't walk better so its all waste so she is skipping her medicines. In this case first of all talk to the grandmother and explain about the disease condition, what is the need for medication and help her to perform simple exercises and walking. Support her and develop trust in her that she can walk. Thus by developing the belief will automatically promote her health.

15. Outcome is the part in the model. It measures whether the intervention result in good or bad outcome.
