This article discusses the management of chronic wounds. Pressure ulcers are common in both acutely hospitalized and long-term institutionalized patients and estimated to occur in approximately 9% of hospitalized patients.In high-risk hospitalized patient, the prevalence as high as 38%.The annual risk of pressure ulcer in patients with neurologic impairment is 5-8%, with lifetime risk of approximately 85% and mortality rate of 8%. Factors adversly affect wound healing are diabetes, infection, drugs, nutritional problems, tissue necrosis, hypoxia and low temperature. Laboratory studies done are complete blood count, metabolic profile to know the renal insufficiency and electrolyte abnormalities, serum albumin, protein prealbumin, and transferrin levels to know the patient's nutritional status, tissue culture and coagulation studies. Vascular ultrasonography, CT, MRI and plain radiography may be helpful in certain conditions.

Treatment includes complete assessment of the patient especially for diseases like diabetes mellitus. Next is to characterize the wound based on size and depth, appearance of wound surface, wound exudate characteristics and amount, status of periwound tissues, ensure adequate oxygenation and nutrition, treat the underlying infections if any, irrigate the wound and provide moist wound bed. Some of the wound dressing materials used are alginate, hydrofibre, debriding agents, foam, hydrocolliod, hydrogel, low adherence dressing and transparent film.

Recent systematic reviews suggest that different debridement methods chemical, surgical or autolytic, all result in similar results.Other reviews suggest that there may be a role for negative-pressure wound therapy and hyperbaric oxygen in the management of chronic wounds and probably there is no role for electromagnetic therapy or low-frequency ultrasound therapy based on current best evidence.
