Case Study: Conduct Disorder Peter, age 16, was picked up by the police and taken to the emergency department of a large urban hospital. Several people had reported his behavior to be erratic. A drug screen indicated that he had been using marijuana and lysergic acid diethylamide. He was identified by a driver’s license in his wallet. His mother was located and notified of his whereabouts. Peter was admitted to the psychiatric unit for detoxification. Peter’s mother reported that she has had little control over him since Peter’s father left with another woman when Peter was about 9 years old. Since that time, Peter has been in trouble with school officials, the law, and his teachers. He has been arrested for shoplifting, possession of illegal drugs, destruction of the property of others, and most recently, when he and some of his friends “hot-wired” a car and went for a joy ride. Another time, the neighbor called police to report that Peter was shooting at his dog with a BB gun. Lately he has been truant from school more days than he attends. The psychiatrist has assigned a dual diagnosis of Conduct Disorder and Polysubstance Abuse. Design a nursing plan of care for Peter.

NURSING CARE PLAN ON POLY SUBSTANCE ABUSE :

NURSING DIAGNOSIS - Ineffective coping pattern related to dysfunctional family as evidenced by unable to verbalize feelings of inadequacy.

GOAL - To make the client to discuss about the major stress in the life

NURSING INTERVENTIONS -

* Motivate the patient to express his feelings which caused him to go for the current situation

* Setting of certain limits on the client to accept his behavior and modify it

* Motivate the patient to document the daily emotions and stress in a diary to keep an eye about his behavior

* Give instructions to the client for coping strategies to overcome from the problem

* Reviewing the client with community resources

EVALUATION - The client improves his communication skills and may change his behavior soon.
