Accurate documentation is an important aspect of a patients chart,as it provides es valuable information regarding the type of care the patient recieved at the time of admissdion to discharge.The tracer methodology looks into patients to assess care and compliance with adequate standards.In tracer methodology a selected patient is followed from admission to dischrge to acess/evaluate compliance with standards.it helps in discovering problematic practices.it helps the surveyors in identifying elements which are missing.if there are missing parts or illegible entries will not result in a successful survey under the new process.Tracer methodology will provide opportunities for improvement and helps in developing an efficient organizational system.
