Needing 3 diagnosis for a patient on an antepartum unit who is 31 weeks pregnant. Admitted for Funneling of cervix. This is the patient's second preterm. GTPAL is 2/0/2/0/0.

Patient is put on bedrest. Patient also stated to weigh 135 lbs and gained ~13 lbs thus far of pregnancy. Patient also has a immediate family member in hospice care. Patient has no medical history. Has had a previous delivery at 21 weeks, demise.

Funneling of the cervix is a sign of cervical incompetence with cervical canal inner part dilatation and it cause cervical length reduction. It is a preterm delivery risk happen at 25weeks.
Nursing diagnosis:
Anxiety:
Anxiety due to the delivery process and family member in hospice care.
Suspected preterm labor:
Suspected preterm labor due to previous delivery at 21 weeks.
Acute pain:
acute pain due to the funnerling of the cervix and potential tissue damage.
