A 65 year old male has been admitted to the hospital with acute cellulitis of the left lower extremity. This is his third admission for cellutlitis .The culture has once again come back showing Methicillin Resistant Steptococcus Aureus (MRSA), consistent with previous admissions. He is being placed on Vancomycin 1g IV BID. The Vancomycin is mixed 1g in 200ml of NS. You are to infuse each dose no faster than 10mg/min. In how many minutes will you infuse the 200ml of Vancomycin? How many ml/hour will you set the pump? What classification is Vancomycin? What are adverse effects and nursing considerations?

Answer :- 1gm = 1000mg

1gram vancomycin is mixed in 200ml fluid means 1ml have 5mg

1ml=1000mg÷200ml

=5mg/ml

Question already suggests not to start not more than 10mg/minute

Means 2ml/minute is the maximum rate we can set to the Infusion pump (for an hour 120ml/hours we can set).

Classification = Glycopeptide antibiotics (branch - tricyclic glycosylate non-ribosomal peptide)

It works by inhibiting of bacterial cell wall synthesis in gram positive bacteria.

Adverse effects = nausea, hypokalemia & abdominal pain are some of the common adverse effects.

Nursing considerations = Nurse should know about the adverse effects and complications or side effects, warning or contraindications like in hypersensitivity we can not give.

In pregnancy avoid use other substitute of this medicine.

Nurses should know how to administer it rapid administration may cause urticaria, flushing, pruritus and hypotension... Etc
