Answer ; Yes, there is a ethical dilemmas in this scenario.

Mr.c is very old man, almost he is 90 years now, he been diagnosed as congestive heart failure. It is well known that his prognosis doesn't have any improvement and it is totally poor. Now he is just visiting the ED for sometime and getting some palliative treatment from the physician. Though he is supposed to get advance treatment and monitoring from the health care provider, he is still getting only symptomatic treatment.

The ethical dilemmas here is the relationship between Mr.C and health care provider. As a health care provider it is their duty to diagnose accurately about the condition of the patient and discuss with him about the treatment and facility. As a old age client he should cooperate with the health care team. In this scenario the client is not yet admitted to the ED.

The reason that client did not admit to the advance treatment might because of their financial condition. There are so many factors that cause the prevent the patient to get proper treatment from the hospital. It may be his culture and beliefs, or it may their economical condition. But as this result Mr, C is only receiving end life care from the hospital in spite of his poor prognosis.
