You are the administrator of the only hospital in a community of 15,000 people in the rural area of a mid-western state. Having read the journals and attended the conferences of your professional associations, you have been inundated in calls for Quality Improvement. You are convinced a QI program should be implemented in your hospital, but you know the members of your board consisting of a car dealer, banker, life insurance salesman, large acreage farmer, and Presbyterian minister are generally opposed to big city management ideas. In addition, physicians in your hospital are often talking about bureaucratic red tape when they have occasion to visit large metropolitan hospitals; in fact, they cite this as the reason they prefer the small town practice. How do you get the board and physicians to support a QI program?

Being a health care provider and the only provider for a large group of people in that area, we always have to be very careful with our care and with the quality of our treatment. It should always be towards the patient's life and should be based on our patient and his health.

After being able to understand about the quality of our hospital, I would never keep quiet and let the hospital go on with its treatment. In order to make our hospital worthy enough to provide quality care to our patients we should implement the quality improvement programme in our hospital. To get the support from the board and the physician for the QI programme I would present my information which I have collected about the ttype of care we are providing to our patients and how it might lead to any harm to the lives of the people. And being the only people to provide care how we should provide all the things required for them in proper amount and level. I would also assure them that this programme will not at all change our hospital's location or the people in it, it will always be the same but with a little improvement in care.
