Mike, 29 years old, was admitted to a community hospital three days ago with weakness and hypertension after sustaining a spider bite while hiking in the woods. Mike has a large hematoma on his left arm where he was bit. He has no prior medical history, no drug allergies, and does not take medication. Mike started to experience Moderat respiratory distress, and started losing blood from his IV sites, nose, and catheter. He is smiling jaundice and his skin is cool. His vital signs include a heart rate of 110 bpm and a regular blood pressure of 92/44, slightly Labor respiratory rate of 22 breaths per minute, and a pulse oximetry reading of 91%. What would you initial diagnosis be, explain? What diagnostic test would you order and why? What would you expect the diagnostic test to show? What is the treatment options for the diagnosis?

#. Initial diagnosis will be brown recluse bite because the symptoms resembles the signs and symptoms of the same spider . Most severe symptoms are :-

Fevers, chills, N&V, myalgia, arthralgia, petechiea, hemolysis, siezures, renal failure, DICC and death

#. Diagnosis is done by colour ,shape , size and by region

#. Treatment :-

1. Application of cold compress (Sphingomyelinase D in the venom is temperature-dependent, so application of local cool compresses may help limit the necrosis)

2. If bite appears infected, application of topical anesthetic under sterile dressing and administration of oral antibiotic

3. Seek advanced medical care for systemic reactions
