Active aging: Active aging which describes the maintenance of positive subjective well-being, good physical, social and mental health and continued involvement in one’s family, peer group and community throughout the aging process.

Healthy Ageing is about creating the environments and opportunities that enable people to be and do what they value throughout their lives. The concept of active ageing is based on three pillars mentioned in the definition are

Healthy Aging:

Healthy aging is about much more than staying in good physical shape and maintaining a sense of purpose and zest for life.

Rights-based approaches: The rights-based approach is a conceptual framework for the process of human development that is normatively based on international human rights standards and operationally directed to promoting and protecting human rights.

Rights‐based approaches are to decision‐making among people living with disabilities, aimed at promoting their greater autonomy and inclusion in civic life. The Convention establishes a right to “legal capacity on an equal basis with others in all aspects of life” and promotes supported decision‐making as a means of accessing this right

In Australia, aged care services are provided for people from 65 years of age (50 years of age for Aboriginal and Torres Strait Islander people), in the community (home care), residential (residential care) and transitional programs.

Consumer-directed care:

Consumer-directed care (CDC) refers to a self-directed healthcare model in which the client is afforded the right to full autonomy in all decision-making related to that care.

Consumer-directed care is based on the following six principles:

1. Consumer choice and control.
2. Rights.
3. Respectful and balanced partnerships.
4. Participation.
5. Wellness and reablement.
6. Transparency.

The CDC care model has only been mandated for Home Care Packages in Australia.

Palliative approach: Palliative care is an approach that improves the quality of life of patients and their families facing the problem associated with life-threatening illness, through the prevention and relief of suffering by means of early identification and impeccable assessment and treatment.

The aim of palliative care is to help you have a good quality of life. This includes

• managing your physical symptoms such as pain
• emotional, spiritual and psychological support
• social care, including help with things like washing, dressing or eating
• help for families to come together to talk about sensitive issues
• support for people to meet cultural obligations
• counselling and grief support
• referrals to respite care services
