There can be certain concerns that might arise such as-  the unelected leaders won’t discharge their duties accordingly; they won’t be accountable to the people. There would be no accountability since they are not elected by the people.

Also, the unelected leaders can’t be removed from their office even though they are they make decisions for the citizens; therefore, they can become reckless when making decisions.

Basically, In the United States, the fourth branch is a term that is used to qualify people that influence or act in the other three branches of government, which includes :

The executive
The legislative
The judicial

Some of the unelected people classified as the fourth branch of government include interest groups, the press, independent administrative agencies, US intelligence community and many more.
