Nursing process for Hip Joint Replacement Surgery

Assessment:

Subjective data:

The patient may verbalise of having pain in hip joint, inability to walk ,unsteady gait, loss of strength while standing and walking around .

Objective data :

The patient may show up with difficulties in walking, anxious and fearful to walk, changes in facial grimace due to pain and discomfort ,hesitating to put pressure on the affected limb

Diagnosis

Impaired mobility related to fractures/loss of function/injury as evidenced by verbalisation and observation

Risk for fall related to affected limb as evidenced by unsteady gait and difficulty to walk

Risk for infection related to incision as evidenced by surgical intervention or hip joint replacement surgery

Implementation

• Pre operative preparation (skin prep, pre-op medications , consents, investigations)
• Administration of medication after the surgery to prevent infection and reduce pain
• Anti coagulant therapy, DVT stocking with pump to prevent deep vein thrombosis
• Physiotherapy for early mobilisation
• Skin care,seconf hourly positioning to prevent bed sore

Evaluation

The patient will be able to walk with the walker and support in about third day.

Improvements in physical activity can be noted by reduced activity intolerance .Able to meet daily activity needs by patient participation in the care.

Regaining the physical strength to walk around .
