Heart attacks sometimes causes what are known as reentrant rhythm disturbances, where the direction of the spread of the cardiac action becomes somewhat chaotic instead of spreading smoothly over the entire myocardium in a single wave. Why does this happen?

This generally happens because of reentry of flow formed because of the slow and fast pathway. These pathways are anteriorly and posteriorly positioned respectively. The circuit revolves around AV node, His bundle, ventricle and atrium. Features like R-P appearance or dominance of slow/fast pathway on ECG confirms the phenomenon.
