A 37-year-old truck driver for Cadbury-Schweppes was found unresponsive in his truck with the motor running parked at a convenience store, where he was making a delivery. EMS brings him to ER.

He is on no listed medications, has not seen a physician for several years, and works on weekends as a soccer coach. He has not seen his doctor for several years. His mother had diabetes onset in her eighties and died after a series of strokes.

On exam: VS:  BP 98/62 mm Hg, pulse 110 per minute, respirations 10 breaths/min.

Gen: Moderately obese, lying in bed, eyes closed, not following commands
  MS: Sleepy, awakens to repeated tactile stimuli, not attentive, no visual tracking
  CNS: Pupils equal 3mm, reactive, EOM seems intact, face symmetric, tongue midline.
  Motor: Limited exam, but he moves all extremities equally and spontaneously.  

  Sensory: grimaces to pin prick in all 4 extremities and face bilaterally.
  Reflexes: DTRs symmetric, 2/4 throughout, bilateral flexor plantar responses, no Hoffman's sign, no ankle clonus
  Gait/Coordination not tested
