The patient information form gives a detailed description of patient information, including all the required details. Types of information that is present on the form is

Demographics include all data related to name, age, sex, address, date of birth, contact information, religion, next of kin, etc. It is also known as identification data, which is unique, and the individual can be traced out.

Past and present illness includes presenting complaints and previous health history, socio-economic data, and psychological data. It helps in understanding the patient physical, mental and emotional status to take the treatment. It also includes any major or minor surgeries the patient may have undergone, which helps plan further treatment.

Treatment:- this includes the type of treatment patients have been undergoing to date for any major health issues so that the right treatment is being provided to the patient. Treatment is not replicated and goes hand in hand with present treatment, if any.
