Lactate is produced by the reduction of pyruvic acid.

H+ is produced by the oxidation of NADH into NAD+ and H+.

Both of these products are formed during fermentation. Fermentation is the process of anaerobic breakdown of glucose. Glucose is first broken down into pyruvic acid by the process of glycolysis. Then pyruvic acid is reduced in to lactate by the action of enzyme lactate dehydrogenase. This is lactic acid fermentation. Pyruvic acid is the final electron acceptor. The electrons are removed from coenzyme NADH to form NAD+ and H+. This leads to the formation of NAD+.

The goal of fermentation is to regenerate the pool of NAD+ so that it can enter aerobic respiration. Fermentation is energetically very poor process because it produces only two molecules of ATP. After NAD+ is reforms, it enters aerobic respiration where one molecule of glucose produces 32 molecules of ATP.
