B. IAPP activates macrophages and induces islet inflammation . Inflammasome activation is the critical event in the initiation of islet inflammation by IAPP, as IAPP induced IL-1β but has no effect on IL-6 or TNF secretion from mmLDL-primed dendritic cells in vitro. The significance of the inflammasome/IL-1 pathway to IAPP-induced islet inflammation in vitro demonstrated by the finding that TNF secretion from macrophages and chemokine secretion from isolated islets induced by long-term stimulation with IAPP was strongly suppressed by IL-Ra. In vivo, IL-1Ra treatment improved glucose tolerance in recipients of IAPP-Tg islets and conferred nearly complete protection from macrophage recruitment to the transplanted hIAPP-Tg islets. IL-1Ra also improved islet function without affecting insulin sensitivity in IAPP-Tg KKAy mice.
