If the cancer cell population is heterogeneous then mutation rate is different with different cells. If a particular chemotherapeutic treatment is resistance for some cells and this cell population mutation rate is high then this cells survive for that particular treatment. This is a natural selection process where highly advance cell is going to survive.

After that treatment, survive cells are are resistant for this chemotherapeutic treatment. This treatment will not going to workout.
